A Providence nursing home with more than $2 million in debt is preparing to shut its doors.

Officials for Bannister House filed a petition Monday in Providence Superior Court for the appointment of a receiver to handle the facility's assets —and the relocation of its residents, according to documents obtained by The Associated Press.
